What Is USCIS Immigration Medical Exam (Form I-693)?

If you file Form I-485, you typically must undergo an immigration medical exam and receive vaccinations against vaccine preventable diseases. USCIS also instructs applicants to bring the most recent edition of Form I-693 and complete Part 1, but do not sign until the civil surgeon tells you to. 

After the exam, the civil surgeon completes and seals Form I-693 in an envelope. USCIS states you must submit the sealed envelope and you should not open it.
